WEBSITE INFORMATION

Your privacy.
A clear explanation.

How this website’s features work, and how to reach the office privately.

This website is not a patient portal.

The website provides general practice and dental-care information. Do not send medical records, insurance identification numbers, payment card details, or other sensitive information through this website. For an appointment or a private conversation about your care, call (909) 655-5050.

Contact and appointment requests

When you choose to submit an enabled contact form, your name, contact details, selected dental topic, patient status, preferences and any scheduling note are sent through the practice’s request service and emailed to dentist@dentistinontario.com using Amazon Web Services. The office uses this information to respond to your inquiry and discuss a visit. A request does not confirm an appointment or change an existing one.

Provide only the information needed to reach you. Do not enter symptoms, diagnoses, medical history, birth dates, insurance identification numbers, payment card details or records. Even an appointment request can reveal information about your dental care. This form is not a patient portal or emergency service. You may call the office instead of submitting a form.

The form does not save drafts in website cookies or browser storage. Until you submit, the information remains in the form in your browser; your own browser may offer autofill. Security checks may contact the request service when you focus the form, but do not transmit your entered contact details until you submit.

The provided backend does not record request contents in application logs or store the full request in its security database. It processes IP addresses for abuse prevention and keeps keyed security identifiers, request status and counters with expiry settings. The delivery workflow temporarily keeps a KMS-encrypted copy of your email address so it can send your acknowledgment after delivery to the office mail server. It removes that encrypted address after processing or expiry. Request records have a 72-hour expiry; security counters have shorter expiry settings, and physical database removal can take additional time. The workflow checks expiry before sending. Operational application logs have a 14-day retention setting. Email delivery services process routing metadata. The acknowledgment queue contains only random request identifiers and delivery status metadata—not your contact details or note.

The request email is retained in the office mailbox and any authorized systems under the practice’s and its providers’ retention arrangements. Email routing, mailbox access and retention depend on the deployed services. The provided email-sending configuration requires encrypted transport to the receiving mail server; it is not end-to-end encryption. Ask the office about its privacy practices or an alternative way to communicate.

The checkbox authorizes one automated acknowledgment email about your request and personal follow-up from the office, not enrollment in marketing or automated text messaging. The acknowledgment is triggered after the office mail server accepts delivery. It does not repeat your selected service, contact details, or scheduling note, and it does not confirm an appointment. Email delivery to a mail server does not mean a person has read the message. For a concern about the information you submitted, call (909) 655-5050.

Email language and scheduling-note translation

The language of the page used to submit your request determines whether your automated acknowledgment is in Spanish or English. The request service keeps this language code with temporary request-status metadata. Office notifications use English labels and choice descriptions. When translation is enabled, only a scheduling note submitted from a Spanish page is processed by Amazon Translate to provide an English machine translation for the office. Name, phone, email fields and the chat conversation are not sent to the translation service. Do not put medical or other sensitive information in the note.

The office receives the labeled machine translation together with the original note. If translation is unavailable, the office receives the original with a warning; a translation failure does not discard the request. Translations can contain errors and do not replace direct communication. AWS providers process the note under the account configuration and applicable policies. Your acknowledgment does not repeat the note or other form details. You can call the office instead of using this service.

Dental-service chat assistant

The automated website assistant matches typed questions to prepared information from this website. It uses common wording, close spellings, and the recent service topic to find relevant answers. When a match is uncertain, it offers related questions or directs you to the office. It does not generate unrestricted answers. The appointment-request guide asks one contact or scheduling question at a time; optional preferences can be skipped. It is not a live staff member, a dentist, a patient portal, or a general-purpose AI service. It cannot diagnose symptoms, prescribe medicine, check live appointment times, or confirm a booking. For urgent dental concerns, call the office; for a medical emergency, call 911.

Questions and draft replies are processed in your browser. The assistant does not send chat questions or a chat transcript to a model provider, the office, or the request API. It does not save the conversation in cookies, local storage, or session storage. The chat can remain in memory while this page is open, even when you close the chat panel. Use “Clear chat” to remove it from the panel, or leave or reload the page. Your browser may provide its own autofill, history, or caching. Do not type private medical details, insurance numbers, birth dates, or payment information into the chat.

When you explicitly start an appointment request, the assistant can use details you have already entered in this page’s form. It asks for the same contact and scheduling information, lets you review and edit it, and requires the contact-permission checkbox and “Send my request” action. Only the reviewed request fields—not the conversation—are then copied into the existing page form and sent through the same AWS email workflow described above. “Continue in page form” copies the draft for your review but does not send it or select consent. Selecting a dental topic does not determine treatment.

Clear chat removes the chat display and its unsent chat draft. It does not erase information already copied into the page form, withdraw an email, or cancel a pending request. After a connection error, the page keeps the same pending request identifier for a deliberate retry. Do not start a second request if the result is uncertain. Contact the office using the displayed reference instead.

Visit guides and browser-only tools

The homepage starting-point guide, visit planner, first-visit guide, and animation controls operate in your browser memory. They do not submit an appointment request or send your selections to the practice. This version does not use cookies, local storage, or a database to retain these selections. Reloading or leaving the page may clear them.

Choosing “copy” places the selected phone number, office address, or call outline on your device’s clipboard. Other applications with clipboard access may be able to read what you copy. The outline does not ask for your name, contact details, or medical history.

Selecting “Save our contact” downloads a contact card containing only the office’s public name, telephone number, and address. Your device may ask whether to add it to your contacts. Nothing from your contacts is sent to this website.

The Google map does not load automatically. Selecting “Load Google Map” requests content from Google; selecting directions opens Google Maps in another tab. Those actions may share information such as your IP address and browser details with Google, whose own privacy practices apply.

The website’s essential images, styling, and scripts are served as part of this site, without remote fonts or a stock-photo feed.

Website hosting and phone calls

A website hosting provider may process technical request information, such as IP addresses, browser details, timestamps, and requested pages, for delivery, security, and administration. This website package does not install advertising pixels or analytics scripts. Additional tools introduced by a host or administrator may change how information is processed.

Clicking a phone link opens your device’s calling service. Information you provide during a phone call is handled by the office and its service providers, not by the visit planner.

Questions about medical-record privacy

This page describes website features only. It is not the practice’s Notice of Privacy Practices for protected health information and does not replace that notice. Contact the office to request the practice’s applicable privacy information or ask questions about your records.
